What are Alli tablets?

Alli tablets are an over-the-counter weight loss treatment containing orlistat 60mg. They work by reducing the amount of fat your body absorbs from food, helping support gradual, sustainable weight loss when combined with a reduced-calorie, lower-fat diet and regular physical activity. Orlistat belongs to a group of medicines called lipase inhibitors, which work inside the digestive system to reduce fat absorption from food. This makes it different from GLP-1 treatments like Mounjaro or Wegovy, which can help reduce your appetite and slow digestion.

How does Alli work?

Alli works by reducing the amount of fat absorbed from meals. The active ingredient, orlistat, blocks digestive enzymes called lipases, preventing some dietary fat from being broken down and absorbed. Studies show that orlistat can block approximately 25% of dietary fat absorption when taken correctly alongside meals containing fat.

Is Alli the same as Orlistat?

Yes. Alli contains the active ingredient orlistat. However, Alli contains a lower strength dose: Alli: orlistat 60mg. Prescription-strength orlistat: 120mg. Both medicines work in the same way by reducing fat absorption, but higher-strength prescription versions may be recommended for people requiring additional clinical support.

How much weight can I lose with Alli?

During clinical studies, Alli helped almost half of patients (47%) lose at least 5% of their starting weight after 6 months.

And even this modest amount of weight loss may improve blood pressure, cholesterol, and blood sugar control.

Clinical studies suggest that people taking orlistat alongside diet and exercise may lose more weight than those using lifestyle changes alone.

Weight loss results vary depending on:

  • starting weight
  • diet quality
  • physical activity
  • consistency of treatment

Alli is designed to support gradual, realistic weight loss rather than rapid changes. However, if you haven’t lost at least 5% of your starting weight by 12 weeks, your clinician may recommend stopping your treatment.

Regardless of how much weight you’ve lost, you should stop taking Alli after 6 months.

Who can buy Alli?

Alli may be suitable for adults aged 18 or over with:

  • a BMI of 28 or above
  • weight-related health concerns

It may not be suitable for everyone.

You should not take Alli without medical advice if you:

  • are pregnant or breastfeeding
  • have chronic malabsorption syndrome
  • have cholestasis (reduced bile flow)
  • take certain medicines that interact with orlistat

A healthcare professional or pharmacist should assess whether Alli is appropriate for you before starting treatment.

Does Alli have any side effects?

Yes. Alli can cause side effects, particularly gastrointestinal symptoms linked to undigested fat passing through the bowel. They’re usually mild, and are most likely during your first 3 months of treatment.

Common side effects of Alli include:

  • oily stools
  • urgent bowel movements
  • flatulence
  • loose stools
  • abdominal discomfort

These side effects are usually more likely if meals are high in fat.

Less common side effects of Alli may include:

  • headache
  • fatigue
  • anxiety
  • changes in bowel habits

When to seek urgent medical help

You should seek medical advice immediately if you experience:

  • severe abdominal pain
  • yellowing of the skin or eyes
  • blood in stools
  • severe allergic reactions
  • persistent diarrhoea
  • signs of liver problems

Long-term use may also affect the absorption of fat-soluble vitamins such as vitamins A, D, E, and K. You should leave a 2 hour gap between taking Alli and any supplementary vitamins.

How to take Alli

Alli should be taken exactly as directed in the patient information leaflet or by your pharmacist.

  • Take one capsule with water.
  • Take during, before, or up to 1 hour after meals containing fat.
  • Skip the dose if the meal contains no fat or if you miss the meal entirely.

For the best results while taking your treatment:

  • follow a lower-fat diet
  • Spread your fat intake evenly across meals
  • stay physically active

How often do you take Alli?

The usual dose is one 60mg capsule, three times daily with meals containing fat.

Do not exceed the recommended dose.

What are the different doses of Alli?

There’s only one dose of Alli available: orlistat 60mg. If you want the higher dose 120mg orlistat tablets, you’ll need to complete a consultation to get a prescription.
