Mounjaro (tirzepatide) is a prescription weight loss medication that may help overweight and obese adults with a BMI of 30 or above or a BMI of 27 and above along with a weight-related health condition to lose weight.
It’s a weekly injection you administer yourself at a time that suits you, giving you the flexibility to fit it into your schedule.
Like other weight loss injections, it reduces your appetite, helping you to feel fuller for longer.
The active ingredient in Mounjaro, tirzepatide, helps you lose weight. Tirzepatide mimics the effects of natural hormones that regulate appetite and food intake.
It targets areas of the brain involved with appetite regulation, leading to reduced hunger and feelings of fullness.
Unlike similar medications, tirzepatide is a GIP receptor agonist and a GLP-1 receptor agonist.
GIP regulates blood sugar and fat metabolism. It helps to encourage the production of insulin in the body and increase insulin sensitivity.
A GLP-1 receptor agonist works to help you lose weight by slowing down gastric emptying. They also act on the hypothalamus, an area in the brain that can control your hunger, which reduces your appetite.
Mounjaro comes in 6 different doses. You will start on the lowest dose before your prescriber gradually increases your dosage (known as titration).
Your dose is increased every 4 weeks, giving your body time to adjust. This gradual approach helps to ensure effective treatment while minimising the risk of unwanted symptoms.
This is the lowest dose available for Mounjaro injections, and you’ll usually start by taking this for the first 4 weeks.
The main purpose of starting on the lowest dose is to avoid experiencing any severe side effects, so please don’t be disheartened if you don’t see much movement on the scales whilst you’re taking 2.5mg.
Some people do start to see some weight loss even on this lowest dose.
Taking 2.5mg injections weekly for the first month of treatment means that your body will be allowed to gradually get used to the medication before you titrate up to a higher dose.
From weeks 5-8, most people move onto a 5mg injection every week.
Although this still isn’t the therapeutic dose, remember that this is still a titration period to allow your body a chance to adjust to treatment.
You should take the 5mg dose weekly throughout your second month of treatment.
From week 9, you’ll be able to take a weekly dose of 7.5mg.
For most people, this is enough to start seeing results, and many people who take weekly injections for weight loss stay at this dosage for as long as they need to.
You shouldn’t ever take 7.5mg straight away without titrating up from a lower dose, as this could cause severe side effects.
The Surge plan is suitable for people who have been taking a 7.5mg dose for a while and have either reached a plateau or stopped seeing as much success as expected.
The higher doses of Mounjaro are ideal for those who have seen success with their treatment so far but could benefit from a stronger dose as treatment continues.
Your prescriber will make the final decision about whether or not a surge plan would be appropriate for you, and if so, how long you should stay on it.
This is the highest dose of Mounjaro, but it’s important to note that not everyone will need to titrate up to this dose in order to achieve effective weight loss.
Mounjaro can help individuals lose up to 8% of their body fat in as little as 12 weeks[1].
A study concluded that on average, people who take Mounjaro lost on average 25% of their body weight in 1 year[2].
Mounjaro should be used alongside a low-calorie diet and exercise for the best results.
Mounjaro starts working immediately. However, it might take several weeks to notice any changes.
Most people notice changes around 4-6 weeks after taking it. Typically, you may see a reduction in appetite and a slight decrease in weight. Significant weight loss usually occurs over 3-6 months[3].
The exact timeline for seeing results can vary. Factors like your starting weight, lifestyle, and how your body responds to the medication can play a role in this.
If you continue to stick to a healthy diet and exercise after Mounjaro, then you shouldn’t regain weight when you stop taking the medication. Hopefully, the healthy habits you have adopted will remain in place.
However, if you stop exercising or exercise less and consume more calories than you did while using Mounjaro, you can expect to regain some weight.
You should inject Mounjaro once a week on the same day. You can use your injection at any time on that day, but it might be a good idea to use it at the same time so that you remember.
If you need to change your day, that’s absolutely fine! Just make sure the time between doses is at least 3 days (72 hours).
Before you use your Mounjaro pen, inspect it to ensure there is no sign of damage. If you notice any issues with your pen, do not use it. Look out for particles in the medication or discolouration. You must also check that the dosage is correct and hasn’t expired.
Start by washing your hands thoroughly to reduce the risk of infection at the injection site. Your prescriber will help you decide where to inject Mounjaro, but you must ensure you do not inject the exact same spot each time.
When you are ready to inject yourself, wipe the inner red seal with a swab. Remove the paper tab from the outer shield, but don’t throw it away. Pull the inner needle shield off and dispose of it.
Turn the knob slowly until you hear two clicks. The dose window shows the extended line, which is the ideal position. You can correct the position by turning the dose knob in either direction until the position you need is in line with the dose indicator.
Hold your pen with the needle upwards and gently tap the cartridge holder to collect air bubbles at the top. Release some of the medicine into the air by pushing the dose knob until it stops, then count to 5 slowly whilst holding the dose knob.
Ensure that the ‘0’ icon is in the dose window, but do not inject yourself yet.
If you don’t see a small amount of medicine come out of the tip, you will need to repeat the priming steps again. Do not repeat the steps more than twice. If you still don’t see any medicine, you must change the pen needle and repeat the steps one more time.
You must follow this process each time you inject yourself.
Turn the dose knob until it stops and the ‘1’ icon is on display. Insert the needle into your skin. You inject Mounjaro by pushing the dose knob until it stops. Count to 5 slowly while keeping hold of the dose knob.
Before you can remove the needle, the ‘0’ icon must be showing. Pull the needle out of your skin. It’s normal for a drop of the medicine to be on the tip of the needle. You must confirm that the ‘0’ icon is on display. You will not have received the full dose if the icon is not on display.
Reinsert the needle and finish your injection if you haven’t received your full dose. Do not redial the dose.
If you notice blood after removing the needle, press lightly on the injection site with gauze or a cotton ball. Do not rub the injection site.
Carefully replace the outer needle shield you kept from earlier. Unscrew the capped needle and put it into a sharps container.
Do not store the pen with the needle attached. This prevents leaking, blocking the needle, and air from entering the pen.
You must dispose of your used needles in a sharps container right after use; you should never dispose of needles in household waste, as this can cause injury and cross-contamination.
Once you have used your monthly Mounjaro KwikPen 4 times, dispose of the pen (without a needle) in your regular household bin.
Exceeding the prescribed dose of Mounjaro will not increase its effectiveness and may lead to severe side effects.
If you have accidentally injected too much Mounjaro, you must seek medical attention immediately.
Symptoms of an overdose can include severe nausea, vomiting and hypoglycemia (low blood sugar).

If you are taking or have recently taken any medication, including medication obtained without a prescription, talk to your GP or pharmacist before using Mounjaro.
Mounjaro should not be used during pregnancy as the effects on an unborn baby are unknown.
If you are pregnant, think you might be pregnant or are planning to have a baby, talk to your healthcare provider before using this medicine.
Contraception is recommended when you are using Mounjaro.
If you are breastfeeding, you must talk to your healthcare provider before using Mounjaro. It is unknown whether tirzepatide passes into breast milk.
Mounjaro should not be used by anyone under 18 years old.
You don’t need to use Mounjaro with food and drink. Some people may experience fewer side effects when they use it before eating, but it’s important to find what works best for you and stick to that routine.
While using Mounjaro, you should include lean proteins, high-fibre foods and healthy fats in your diet. Try to avoid foods that are high in saturated fat, refined carbohydrates, and processed foods.
Most supplements are fine to take alongside Mounjaro; however, we don’t advise protein shakes, energy drinks or other workout supplements. You can learn more about supplements while on Mounjaro here.
Alcoholic beverages can contribute to weight gain due to their high-calorie content and should be avoided while on your Mounjaro treatment plan. Drinking alcohol can also lead to dehydration, disrupt your sleep, and hinder your weight loss progress, potentially affecting how Mounjaro works.
Additionally, consuming alcohol while using Mounjaro may increase the risk of developing other health issues or life-threatening conditions.
You must keep Mounjaro out of sight and reach of children.
Do not use this medicine after the expiry date which is stated on the pen label and on the carton after ‘EXP’. The expiry date refers to the last day of that month.
Store in a refrigerator between 2°C–8°C. Do not freeze the pen. Store it in the original packaging to protect it from light. Mounjaro can be stored unrefrigerated below 30°C for up to 30 days, and then the pen must be discarded.
